What is Breast Augmentation?

Breast Augmentation provides women with beautiful, full curves.

Breast augmentation is a surgical procedure done to enlarge and restore the size, as well as modify the shape of the breast. It is also referred to as augmentation mammoplasty, or breast implant surgery or breast enhancement. A Breast augmentation procedure is performed by placing an implant either behind the existing breast muscle or in front. This pushes the breast outward to give a larger, fuller, firmer and shapelier breast contour. The procedure can also be performed without an implant, utilizing the individual’s own fat as a fat transfer procedure. This type of augmentation is currently considered experimental. It has shown good results in specific types of situations where volume requirements are small.

When selecting the type of implant, a review of chest anatomy, body type, visual preferences and the specific goals that you are seeking from the augmentation will all be part of your discussions.

Implant decisions will include:

  • Implant Material: the type of the implant material best suitable to address the current small breast size for the longest lasting result;
  • Implant Size: the most optimal size to achieve the desired projection – overstated, dramatic, or subtle
  • Implant Location: the location of where the implant will lie-referred to as breast implant pocket placement – which can be above or below the pectoral muscle.
  • Implant Incision: the location of the incision in the breast area through which the implant will be inserted and positioned.

Breast Implant Choice: Silicone or Saline?

If you are a candidate for either type of breast implant material then you have a choice. On the other hand, your best option may only be one type. Dr Makhlouf will help navigate the choices and provide you with his recommendations based upon a thorough evaluation of your breast tissue, fat and chest structure.

Breast Implant Choice: Silicone or Saline?

Silicone Implants: By far the most popular material used in implants are silicone – employed in 85% of breast augmentation procedures. Safe and FDA approved for breast augmentation for over 22 years of age, silicone implants come as individual pre-filled shells with silicone gel measured in definitive amounts. They come in a wide range of sizes. This is important for those seeking a variety of larger sizes to choose from. Silicone implant shells require larger incisions for placement of the implant. A silicone implant may feel softer and more natural than saline implants and tend to have less projection. Today’s silicone implants are vastly superior to their predecessors with significant technological strides made; the 7th and 8th generation of silicone implants are now available. The outer shells have become progressively thicker and more durable to greater withstand ruptures and the silicone gel has improved in terms of viscosity and density. If your priority is a more natural appearance than silicone may be the implant of choice for you.

Saline Implants: These implants are safe and FDA approved for breast augmentation. The implant shells are not pre-filled. They are carefully filled with a solution of sterile salt water during the breast augmentation surgery and proportioned for a balanced result. Saline implants require smaller incisions than those used for silicone implants, as well as affording more flexibility in the location of the incision. Saline implants tend to be firmer and more supportive than silicone with increased projection. Saline implants provide a cost effective option for breast augmentation

Breast Implant Sizes

The implants used are round, with varying levels of projections – the amount the implant extends outwards from the chest. During your consultation you will be guided through the different sizes and firmness of implants which provide for different projections or profiles.

With the use of external sizers, implants of various sizes and firmness are placed under a sport bra. This gives a very good idea what each implant will do for the overall contour of the chest.

Breast Implant Incisions

For a breast augmentation procedure using implants each meticulous incision becomes the entry point for the breast implant.

There are three incision locations: 1) inframammary or breast fold; 2) peri areolar,  3) trans axillary.

Each approach carries advantages and disadvantages in terms of approach, scar visibility and optimal implant positioning.

Inframammary: Underneath the curve of the breast, barely above the crease or fold of the breast – the inframammary fold – provides for a less conspicuous scar and is the most commonly used. This approach provides the best method for optimal surgical control over positioning of the implants and is associated with fewer breast-feeding problems.

Peri-areolar: Around the lower edge of the areola –- tends to result in a less conspicuous scar hidden in the darkened areola area. A sufficient sized areola is important for this option. This approach is associated with increased firmness and a slightly higher chance of partial or total loss in nipple sensation.

Trans axillary: The incision is located the lower armpit and provides for no scar in the breast region. This approach is less optimal for cleavage definition. There are also limitations in placement when larger breasts implant sizes are required.

The most popularly chosen incision

is the one concealed in the breast fold

Breast Implant Location

Your customized plan will outline the ideal breast implant pocket location. You will be guided as to the advantages and disadvantages to having the breast implant placed in the different pocket locations. An evaluation of the unique characteristics of your breast skin and tissue will be among the factors to help determine the pocket location.

Sub glandular location: The implant is placed above the breast muscle and below the breast gland. It tends to be a less invasive approach with potentially a speedier recovery. This approach with the implant placed nearer to the skin’s surface, will require sufficient breast tissue to camouflage the implant so it is not as visible. Advantages: With the implant location intimate with the breast gland, as gravity inevitably claims its course, the implant will follow the breast tissue. This is an important consideration for women who have a B cup or larger breast.

Submuscular Location: When the implant is placed underneath the chest muscle, this is a more invasive approach with a slightly longer recovery timeline. Advantages: This submuscular or subpectoral approach, creates natural looking results and reduces risks of visible wrinkling and rippling. This location can also provide for more fuller volume to the upper slope of the breast, called the upper pole.

Why Choose Dr Makhlouf Office for Your Breast Augmentation Procedure

Dr Makhlouf is one of Chicago’s skillful, technique-oriented and compassionate plastic surgeons and skillfully adept at enhancing breasts for many reasons – including the following:

During surgery, the employment of the technique of “Volume Sizing” affords the opportunity of a breast sizing preview before final implantation Dr Makhlouf will assess the appropriate implant volume for each patient with a specialized device – a sterile sizer. The sizer is designed for temporary placement in the surgically-prepared breast pocket, to preview and evaluate the appropriateness of the volume size of the breast implant, chosen during the consultation. Only then are the final implants inserted and placed in position.

Dr Makhlouf practices the “No-Touch” technique. This involves changing gloves for new ones that have not touched the patient skin to minimize the chance of contamination. It also involves the use of antibiotic and saline solutions before the implants are touched to check that there are free of manufacturing defects. The implants are touched by only Dr. Makhlouf under his strictest surgical protocols. The implant is delivered into the pocket created, without ever touching the patient’s skin (the “No Touch”). This technique decreases the risk of bacterial contamination, thereby decreasing the body’s reaction, manifested by scar formation. This reaction is called capsular contracture.

Use of the “Patient Upright Method”. Dr. Makhlouf believes that breast pocket is best evaluated while the patient is in an upright position. Therefore, during the course of surgery, by sitting the patient up while asleep, and with the sizers in place, Dr Makhlouf is able to check for symmetry and adequacy of the space created for the permanent implants. The “Patient Upright” method is safe and highly effective.

Q & A on Breast Augmentation

Question and Answers

Q-How long after a Breast Augmentation will I see results?
Immediately following surgery, you will be able to observe a change in your body contour owing to the resulting increased projection in the breast area. Although initially the breasts may look higher placed in terms of positioning, each implant will take time to settle into a more optimal position as the regional swelling subsides. You may expect final results between approximately 3-5 months.

Q-Does a Breast Augmentation procedure involve anesthesia?
Yes, breast augmentation involves general anesthesia or localized anesthesia and is performed on an outpatient basis meaning you are able to go home following your surgery.

Q-How long will my breast implants last?
Both silicone and saline implants are now supported with a free limited warranty and a free lifetime replacement warranty in the event they are found to be defective in any way. It is not reasonable to expect that a man-made device and procedure will match the lengthening life expectancy. Therefore, it is to be understood that some “maintenance” may be needed. Implants may need to be replaced for a different size or removed altogether and skin re-draping may have to be considered.

Q-Will breast feeding my baby be affected if I get breast implants?
No, breastfeeding remains largely unaffected when a mother undergoes breast augmentation. The surgical approach does not interfere with any of the milk production aspects, and the ducts connecting the gland and the nipples are not damaged. Silicone is not believed to leak in the milk to any measurable degree.

Will I lose any nipple or overall sensation following breast augmentation?
85% of patients in U.S who undergo breast augmentation surgery do not experience any change in nipple sensation. A minority of patients – 15%- may be experience a temporary decreased nipple sensitivity, numbness in the nipple area or even a heightened nipple sensitivity and in a small minority – total loss of nipple sensation. This will largely depend upon whether the areolar incision approach was selected and the size of the pocket area created. The larger the breast pocket needed for accommodating the implant, the more dissection is needed. This carries an increased risk of affecting the nerves.

Q- In what places will I have bruising?
With the popularization and routineness of breast augmentation – it is important to remember that plastic surgery is a surgery- you can expect your breasts to bruise and swell as they heal and the extent will vary from person to person depending on the usual skin response of the individual.

Q-Will I have any scars?
Incisions will be made to serve as the entry point for your selected implant and are usually carried out in any of the three typical locations: around the areola, in the inframammary crease below the breast or in the armpit. These specific incision sites are relatively inconspicuous, and will diminish over time, becoming barely visible. Incision recovery will depend upon the body’s overall natural healing response and close adherence to your individualized post-operative recovery guidelines.
